Period Late But Not Pregnant — 9 Real Reasons Your Cycle Is Delayed

You've taken the test. It's negative. So why is your period late?

A late period with a negative pregnancy test is one of the most common cycle questions — and the answer is rarely “something is seriously wrong.” Your period is late because ovulation was late, delayed, or didn't happen at all. And ovulation responds to an enormous range of signals from your body and your life.

Here are the nine most common reasons, and what each one actually means for your cycle.

1. Stress

Stress is the most common cause of a delayed period, and it works through a specific mechanism. High cortisol disrupts the signalling between your brain and your ovaries — specifically, it suppresses GnRH (gonadotropin-releasing hormone), which is the signal that kicks off the hormonal cascade leading to ovulation.

When ovulation is delayed, your period follows it later. The frustrating part: the stress of a late period can further delay it. Many women notice this pattern — a big life event pushes their cycle out by one to two weeks, then it normalises the following month.

2. PCOS

PCOS is the most common medical cause of irregular periods and missed cycles. Elevated androgens and insulin resistance disrupt the normal hormonal sequence needed for ovulation. Without ovulation, there's no progesterone surge — and without that surge and subsequent drop, there's no trigger for menstruation.

PCOS cycles can range from 35 to 90+ days, or be absent for months. If you're regularly having cycles this variable and pregnancy has been ruled out, PCOS is worth investigating with your doctor. Blood tests and an ultrasound are typically used to confirm the diagnosis.

3. Thyroid issues

Both hypothyroidism (underactive) and hyperthyroidism (overactive) can affect your cycle. The thyroid regulates metabolism across the entire body — including the hormonal processes that govern ovulation and menstruation.

Hypothyroidism tends to cause longer, heavier periods or missed periods. Hyperthyroidism can cause shorter, lighter periods or amenorrhea. If you have other thyroid symptoms — fatigue, weight changes, temperature sensitivity, hair changes — thyroid function is worth checking with a simple blood test.

4. Significant weight change

Rapid weight gain or loss can disrupt the hormonal balance your cycle depends on. Fat tissue plays a role in oestrogen production — too little body fat reduces oestrogen below the threshold needed for a normal cycle. But significant weight gain can also alter the hormonal environment, particularly if it involves changes to insulin sensitivity.

The threshold differs between individuals, and the effect isn't always immediate. A cycle delayed by several weeks often reflects something that happened a month or more ago.

5. Over-exercising

Intense exercise, particularly combined with insufficient calorie intake, can suppress ovulation through a mechanism called low energy availability. When your body doesn't have enough energy to support both physical performance and reproductive function, it deprioritises reproduction.

This isn't just a concern for elite athletes — it can happen with marathon training, sudden increases in exercise intensity, or any pattern where energy expenditure significantly outpaces intake over an extended period. The resulting condition, sometimes called the Female Athlete Triad, often presents as irregular or absent periods.

6. Coming off hormonal birth control

After stopping hormonal contraception — particularly the pill — it can take one to three months for your natural cycle to reassert itself, and longer for some women. Hormonal contraception suppresses your natural ovulation; once you stop, your body needs time to restart that process.

This is often called post-pill amenorrhea, though it applies to other hormonal methods too. Most women see their cycle return within three months. If it hasn't regulated after six months, it's worth speaking with your doctor to rule out an underlying cause that the pill may have been masking.

7. Perimenopause

If you're in your 40s and your cycles are becoming less predictable, perimenopause is a real possibility. The hormonal transitions of perimenopause can begin a decade before your final period — often characterised by cycles that lengthen, shorten, skip, or arrive with completely different flow characteristics than before.

A late period in your 40s that would have been unusual in your 30s is often a perimenopause signal rather than a problem. The pattern over months tells you more than any single cycle.

8. Illness or travel

A significant illness — particularly one involving fever — can delay ovulation. The body's stress response to infection activates some of the same cortisol pathways that emotional stress does, temporarily disrupting the hormonal cascade.

Travel across time zones also disrupts circadian rhythms, which are connected to the hormonal signalling that drives ovulation. A two-week international trip can delay a cycle by a week or two in women who are sensitive to these disruptions. This usually normalises within the following cycle.

9. Low body weight

Below a certain threshold of body fat, oestrogen production falls too low to sustain a normal cycle. The hypothalamus — the brain region that kicks off the hormonal cascade for ovulation — effectively goes offline when energy stores are insufficient.

This is the body's protective response to a perceived famine state. It's also a signal that warrants medical attention, particularly if you're not trying to maintain a low weight or if the absent period is accompanied by other symptoms.

When you track what was happening in your life the month before a late period — stress level, travel, illness, weight changes, exercise intensity — you start to see which factors are most relevant to your cycle specifically. That knowledge is genuinely useful. It's also the kind of thing your doctor needs.

What to note when your period is late: how many days late, what was different about that month, any other symptoms that appeared (unusual discharge, pelvic pain, fatigue, mood), and how the period eventually arrived. That log builds over time into something meaningful.

When to see a doctor

A single late period with a negative pregnancy test, no unusual symptoms, and an identifiable trigger (stress, travel, illness) usually doesn't require a medical appointment. If it normalises the following cycle, you've likely found your answer.

See a doctor if: your period is regularly late or absent across multiple cycles; a late period is accompanied by pelvic pain, fever, or unusual discharge; your cycle hasn't regulated six months or more after stopping hormonal contraception; or you have other symptoms suggesting thyroid issues or PCOS.

Bringing a symptom log — even a few months of data — to that appointment significantly improves the conversation. A GP who can see the pattern has far more to work with than one who's hearing about it for the first time.
